Recognizes and reported abnormalities and/or changes in patients' health status to case manager Assist clientele with personal cares such as grooming and hygiene Perform light housekeeping , cooking, and home maintenance as needed Provide transportation to clients for appointments , grocery shopping, and personal errands Documents residents behavior in terms of mental status , sleeping, and eating patterns in medical record books Monitor and assisted residents through individual service plans modulated by the facility
