Perform Physical assessment within the scope of my nursing license. Take vital signs (blood pressure, oxygen saturation level, and pulse) record and report if outside of normal limits. Medication reconciliation with medical providers and pharmacy Assist with medication set-up weekly or monthly in a pill planner Attend medical appointments as an nurse advocate and discuss plan of care with client and family members Assist with coordinating care and scheduling medical appointments Provide transportation to and from local appointments (non emergency medical transportation) Run local errands as needed Be on-call for client emergencies and/or provide respite care (short term) Provide pet care if needed. Client must sign a HIPAA form as to who can receive protected health information (PHI) as outlined by HIPAA laws.
